For potential subject s we initiate the referral and discuss candidate suitability with academic investigators prior to having patients travel to the site.
For patients with severe progressive disease, lung transplantation becomes the only treatment option remaining. We help patients understand the timing and appropriateness of transplant and help guide them through this difficult process. Bronchoalveolar lavage done during bronchoscopy can help differentiate the patterns of inflammation and exclude infections. Generally, bronchoscopic biopsies are of limited value owing to the small tissue sample size. Open lung biopsy done via Video Assisted Thorascopic surgery provides the gold standard for pathologic diagnosis of ILD. We collaborate with internationally renowned pulmonary pathologists.
Treatment decisions are not always clear cut, owing to the difficulty of precisely diagnosing the underlying condition, the toxicity of the medications and in many cases the ineffectiveness of current therapies. We help guide patients through these important considerations and collaborate with other specialists.
There is a growing pipeline of new medications that are making their way through clinical trials offering new hope for IPF patients. In October of 2014 two new drugs, Esbriet (Pirfenidone) and Ofev (Nintedanib), gained FDA approval, beginning a new era of therapy. These drugs are expensive and carefully regulated making access more difficult. Our Nurse Coordinator will work with you to help you through the process and successfully begin treatment and stay on medication.
