Value Based Program
Overview
The Remote Patient Monitoring (RPM) program utilizes digital technologies to monitor patients outside traditional healthcare settings. Monitoring devices such as blood pressure cuffs, weight scales, and pulse oximeters are used to track a patient's conditions. Each blood pressure reading, weight measurement, or oxygen saturation reading will be transmitted to a healthcare professional to review. Once the data is collected and reviewed, the healthcare team can then provide updates to physicians on their patient’s progress and send urgent notifications when patients are at risk of complications.
Eligible Patients
Patients with the following conditions may benefit from the RPM Program:
- Chronic heart failure
- Diabetes
- Obesity Pulmonary Fibrosis
- Pulmonary Hypertension
- Hypertension
- Hypotension
- Renal Failure
- Chronic Obstructive Pulmonary Disease
Remote Patient Monitoring Team
Referring Physician:
Your PCP or specialist must refer you into the program. They will receive monthly reports on how you are doing and will be contacted if your condition worsens.
Health Coach:
Healthcare professional that frequently monitors and reviews patient’s data. The health coach is also responsible for alerting the referring physician if necessary
