Value Based Program
Overview
This program aims to enhance the typical primary care team to include a Behavioral Health Manager and a Psychiatric Consultant. In the program, the patient will receive monthly contact from the Behavioral Health Manager to help the patient reach their mental health goals. The Behavioral Health Manager will assess a patient's treatment adherence, tolerability, and clinical response. Throughout the program, the referring doctor will be updated by the Behavioral Health Manager and the Psychiatric Consultant on the patient’s progress.
Patient Diagnosis/Eligibility
Patients with the following behavioral health conditions may benefit from the CoCM Program:
- Adjustment Disorder
- Bipolar Disorder
- General Anxiety
- Major Depressive Disorder
- Agoraphobia
- Panic Disorder
- Post Traumatic Stress Disorder
- Psychoactive Substance Abuse
- Psychotic Disorder
Psychiatric Collaborative Care Team
Referring Physician:Your PCP or specialist must refer you into the program. They will receive monthly reports of your progress and will work with the behavioral health team to update or introduce medications if needed.
Behavioral Health Manager:Certified therapist that will work with patients on a weekly, every other week, or monthly basis via telehealth or phone calls. Will draft monthly reports and review patient cases with the Psychiatric Consultant when necessary.
Psychiatric Consultant:Psychiatric Nurse Practitioner that will review patient reports with the Behavioral Health Manager and consult on cases if necessary.
